- Normal Sinus Rhythm (NSR)

Rhythm: regular
Rate: 60-100 bpm
P waves: normal in configuration; precede each QRS
PR: normal (0.12-0.20 sec)
QRS: normal (< 0.12 sec) - Sinus Tachycardia

Rate: 100-150 bpm
P waves: normal in configuration; precede each QRS

QRS: normal (< 0.12 sec) - Premature Atrial Contraction (PAC)

Rhythm: irregular owing to premature beat and pause
Rate: usually normal but can occur with sinus brady or sinus tach
P waves: of PAC is premature & abnormal in size, shape or position (usually small, pointed & upright, but can be inverted); often found hidden in preceding T wave distorting T wave contour
PR: Usually normal but can be short or prolonged
QRS: normal (< 0.12 sec) - Nonconducted PAC

Rhythm: irregular owing to premature P and pause
Rate: usually normal but can occur with sinus brady or sinus tach
P waves: P wave of nonconducted PAC is premature, abnormal & often found hidden in preceding T wave distorting the T wave contour
PR: absent; impulse not conducted to ventricles
QRS: absent; impulse not conducted to ventricles - Paroxysmal Atrial Tachycardia (PAT)
