- to live independtly in a transitional program
- Long-term goal
- to participate in community reentry with significant others
- short-term goals
- -identify accessible community leisure programs, -develop leisure skills compatible with those of signifcant others -to develop community travel and safety skills
- What/How does one measure outcome? (outcome measures)
- look at patient's progress in five domains: cognitive, social, emotional,physical, psychological, and spiritual
- What is the cognitive outcome measure?
- after two weeks in RT, client will identify at least two criteria of accessible community resources
- social outcome measures
- -after two weeks in RT, the client with take fifteen minutes with one caregiver about two or more mutual leisure interests
- Physical outcome measures
- during community reentry program, the client will ambulate using a wheelchair for at least ten minutes with only two rest stops
- psychological outcome measures
- prior to discharge, the client will demonstrate adjustment to the acquired disability by verbally describing two adaptations made to complete a leisure activity during a community reentry experience from the hospital
- Spiritual Outcome Measures
- prior to discharge, the client will verbally describe the significance of the adapted activity as a positive experience to life satisfactior and quality of well-being
- Agency-Wide or Deparment-Level Statements (What is their purpose?)
- -providing comprehensive TR program through experiences that benefit client's health and well-being by impacting cognitive, social, physical, psychological, spiritual, and leisure functioning, support the individual plan, and enable clients to attain functionally appropriate levels of health, leisure, well-being, and autonomy
