Normal Sinus Rhythm (NSR)
Rhythm: regular
Rate: 60-100 bpm
P waves: normal in configuration; precede each QRS
PR: normal (0.12-0.20 sec)
QRS: normal (< 0.12 sec)
Sinus Tachycardia
Rhythm: regular
Rate: 100-150 bpm
P waves: normal in configuration; precede each QRS
PR: normal (0.12-0.20 sec)
QRS: normal (< 0.12 sec)
Sinus Bradycardia
Rhythm: regular
Rate: 40-60 bpm usually (may be slower)
P waves: normal in configuration; precede each QRS
PR: normal (0.12-0.20 sec)
QRS: normal (< 0.12 sec)
Sinus Arrhythmia
Rhythm: irregular
Rate: 60-100 bpm usually but is fairly common w/ sinus bradycardia
P waves: normal in configuration; precede each QRS
PR: normal (0.12-0.20 sec)
QRS: normal (< 0.12 sec)
Sinus Arrest
Rhythm: irregular during pause
Rate: usually slow (40-60) but may be normal
P waves: absent during pause
PR: absent during pause
QRS: absent during pause
* The duration of the pause IS NOT an exact multiple of the other RR intervals
Sinus Block
Rhythm: irregular during pause
Rate: usually slow (40-60) but may be normal
P waves: absent during pause
PR: absent during pause
QRS: absent during pause
* The duration of the pause WILL BE an exact multiple of the other RR intervals
Wandering Atrial Pacemaker
Rhythm: usually regular
Rate: usually normal (60-100) but may be slow
P waves: vary in size, shape & position across rhythm strip
PR: may vary slightly depending on the changing pacemaker location
QRS: normal (< 0.12 sec)
Premature Atrial Contraction (PAC)
Rhythm: irregular owing to premature beat and pause
Rate: usually normal but can occur with sinus brady or sinus tach
P waves: of PAC is premature & abnormal in size, shape or position (usually small, pointed & upright, but can be inverted); often found hidden in preceding T wave distorting T wave contour
PR: Usually normal but can be short or prolonged
QRS: normal (< 0.12 sec)
Nonconducted PAC
Rhythm: irregular owing to premature P and pause
Rate: usually normal but can occur with sinus brady or sinus tach
P waves: P wave of nonconducted PAC is premature, abnormal & often found hidden in preceding T wave distorting the T wave contour
PR: absent; impulse not conducted to ventricles
QRS: absent; impulse not conducted to ventricles
Paroxysmal Atrial Tachycardia (PAT)
Rhythm: regular
Rate: 150-250
P waves: abnormal P waves precede each QRS; seldom identified b/c they are hidden in the preceding T wave
PR: not measurable
QRS: normal (< 0.12 sec)
